As a self-pay practice, we are considered "out-of-network" for most insurance plans. While we don’t accept insurance directly for most services, we do offer a detailed bill that you can submit to your insurance for possible reimbursement based on your out-of-network benefits. We strive to make our care accessible and convenient. You can pay using cash, check, HSA/FSA cards, or choose from various payment plans like Care Credit, Cherry, and AfterPay.
We completely understand that life happens! If you ever need to cancel or reschedule, we kindly ask for at least 8 hours’ notice so we can offer that time to another patient in need.
After two late cancellations (less than 24 hours' notice) or missed appointments, a $15 deposit will be required when rescheduling. This deposit will go toward your next visit—no extra fees, just a little accountability to help keep things running smoothly.
Before your first appointment, please complete all new patient intake forms. Bring a list of all medications and supplements, including prescriptions and over-the-counter items, as well as any relevant health records.
Lab tests and imaging may be needed to identify the root cause of your health concerns. If your provider orders these, please note that they are not included in your appointment fee. Most imaging and some lab tests may be covered by insurance, so check with your plan provider for details. For functional medicine labs, which are often considered elective, fees can range from $250-$550. We will inform you of any associated costs before proceeding with any tests.
